ABSTRACT

Objective To regulate standardized ICD-10 case classification name and coding, and common clinical diagnosis name or expression of mapping rules,to systematically improve the quality of DRGs key data, and to assess the impact of medical diagnostic data quality on DRGs and the indicators based on the DRGs.Methods Extension of the glossaries of clinical diagnosis synonyms or near-synonyms,and establishment of a standardized maintenance procedure of ICD-10 dictionary.Adjustment of the impact extent of DRGs disease makeup on case classification, comparison of the consistency of principal diagnosis classification, and the consistency of DRGs grouping,as well as changes of such indicators as DRGs grouping reduction in variance (RIV) and case mix index (CMI).Results Data of the obstetrics, gynecology and pediatrics disciplines of a maternity and children hospital from 2012 to 2013 (72 005 cases)and 2014 to 2015 (77 705 cases) were chosen for prior-after comparison. The encoding consistency rate was 59.60% before the improvement, with the improved standardized consistency rate rising to 66.38%afterwards;beforehand the DRGs grouping consistency rate was 69.30%,with the improved standardized consistency rate rising to 88.00% afterwards;beforehand the cost RIV was 0.515,with the cost RIV rising to 0.576 afterwards;the CMI variations of individual campuses of healthcare institutions appear more reasonable. Conclusions Diagnostic quality control and improvement project can improve the data accuracy of coding. This empowers the RIV and CMI indexes calculated on such basis to better describe the complexity of clinical settings, conducive to establishing a value-oriented prepayment system which is more transparent, fair and reasonable.

ABSTRACT

From the perspective of both doctors and patients, the questionnaire survey was used in 1080 medi-cal workers and patients in 12 hospitals in Shandong province. They found that is extremely development trend to the doctor-patient relationship, doctor-patient trust evaluation is relevant, doctor-patient both pay attention to the influence factors of the government and society. Both doctors in the building of doctor-patient trust, is atta-ched importance to strengthen the construction of medical ethics of medical personnel, continue to deepen the re-form of the medical system and government regulatory functions into full play.

ABSTRACT

BACKGROUND:There are less studies on static magnetic field, and its biological effects are stil unclear. OBJECTIVE:To investigate the static magnetic field effect on the secretion of nerve growth factor from Schwann cel s. METHODS:Schwann cel s were randomly assigned into three groups:0.05 mT sciatic magnetic field group, 0.1 mT sciatic magnetic field group and control group. At 2 days of inoculation, the cel s were exposed to the sciatic magnetic field, 12 hours per day. Cel s in the control group were not exposed to the sciatic magnetic field. After 6 days of exposure, RT-PCR method was used to detect nerve growth factor mRNA expression in Schwann cel s, and ELISA was adopted to detect the level of nerve growth factor secreted from Schwann cel s. RESULTS AND CONCLUSION:The mRNA expression and secreted level of nerve growth factors in the culture supernatant were significantly higher in the two sciatic magnetic field groups than the control group (P0.05). These findings indicate that the static magnetic field with certain intensity can promote the secretion of nerve growth factors from Schwann cel s.

ABSTRACT

Objective To investigate the in vitro susceptibility of Minocycline and polymycin B against clinical islates of pan-drug resistant Acinetobacter baumanii ,to provide laboratory support for clinical treatment for drug selection. Methods The susceptible test of Minocycline and polymycin against 39 isolates of pan-drug resistant Acinetobacter baumanii were determined by K-B meth-od.Results 38 strains of pan-resistant Acinetobacter baumanii were sensitive to polymyxin B,sensitive rate was 97.4%,20 strains sensitive to minocycline, the sensitivity rate was 51.3%.polymyxin B sensitivity was more sensitive than Minocycline (P < 0.05). Conclusion Polymycin B had strong activit ies against pan-drug resistant Acinetobacter baumanii .

ABSTRACT

Objective To assess the application of spiral CT angiography in the diagnosis of Takayasu arteritis. Methods Vascular imaging features of three-dimensional CT angiography of 56 patients with Takayasu arteritis were analyzed retrospectively. Results The vascular imaging of all the patients were qualified for diagnosis.A total of 175 arteries were involved, including subclavian arteries (36.00%,63/175) with a high percentage of the left subclavian arteries (24.00%,42/175) ,common carotid arteries (29.71%,52/175) and renal arteries (10.86%,19/175) .The affected artery wall had varying degrees of concentric or eccentric thickening, and stenosis or occlusion of lumen was observed. Conclusion Threedimensional spiral CT angiography can make clear diagnosis of Takayasu arteritis, including the scope, extent and nature, and thus is worthy of clinical application.
